ACCREDITATION INFORMATION

Before you can apply for CME credits for physicians, nursing credit hours, or contact hours for pharmacists, you must complete and submit an online Evaluation Form. Please click on the link below to access the survey:

Once you complete and submit your online Evaluation Form, you will be given directions on how to access our PIM's website (our CME provider). There you will be asked a couple of CME-related questions, then upon completion, you will be given immediate access to a Certificate of Attendance. The Certificate will reflect the number of CME credits, credit hours, or contact hours you have earned. Remember to save the Certificate to your personal files as a record of your participation in the HTRS 2017 Scientific Symposium.  Should you have any issues accessing PIM's website, please email HTRS@talley.com to be sent instructions again.
